Falls of the hospitalized patient continues to be a challenge. Falls can result in injuries and complications, longer length of stay, increased costs, and psychological distress. Hospital falls occur because of complex risk factors related to the patient, environment, communication, and system processes. While tools and interventions are crucial in fall prevention, high-reliability organizations use the opportunity to learn from fall events and promote a safe, just culture for staff to participate in post-fall processes effectively. Exploring the types of error and causes that lead to falls can assist hospitals to implement improvement plans. This session will share how a hospital system learns from fall events using post-fall huddles and root cause analysis and how to translate findings into practice change. The session will highlight methods to motivate and sustain staff engagement with fall prevention and a safety culture.

Betty Lee, MN, RN, CNS, CMSRN is a Clinical Nurse Specialist at UCLA Health with over 30 years nursing experience working in the acute care setting. Betty oversees nursing care delivery for the adult medicine patient population at Ronald Reagan-UCLA Medical Center and UCLA Santa Monica. She provides expertise and support to frontline staff to assess and improve system processes and lead practice changes to ensure that nursing care is evidence-based, safe, high-quality, and cost-effective to achieve the best possible patient outcomes. Among her roles, Betty leads the fall prevention program by collaborating with staff and unit leaders, leading fall champions in professional governance councils, reviewing fall metrics, and promoting fall prevention practices. Betty received her BSN from Creighton University and Master of Nursing from UCLA. She is a member of the National Association of Clinical Nurse Specialists, Academy of Medical Surgical Nurses, and Sigma Theta Tau International Honor Society of Nursing. She has published several peer-reviewed articles related to the CNS practice and medical-surgical nursing.
