Achieving Excellence with Value-Based Care: Cedar-Sinai Orthopaedic Superbundles Report

Since January 2021, Cedars-Sinai Medical Center has participated in the Orthopaedic Bundled Payments for Care Improvement Advanced (BPCI-A) Medicare Superbundle. To maintain quality while improving value metrics, the Ortho team examined all aspects of the 90-day episode-of-care for ortho trauma and elective joint replacements. We found that multidisciplinary coordination and collaboration are vital for outstanding patient outcomes, quality, and efficiency. Our focus on patient experience and outcomes has improved fiscal accountability, enhanced value, and transitioned us from a penalty to a bonus within the BPCI-A.

Breda will share the implementation of Cedars-Sinai’s value-based care initiative, including understanding baseline metrics, defining improvement goals, and developing tools like a navigator-driven preoperative evaluation program and an enhanced recovery after surgery protocol. These efforts reduced skilled nursing and acute rehab utilization, stabilized hospital LOS, and decreased ED visits and readmissions. Breda will discuss the initiative’s impact on Cedars’ culture and patient expectations, challenges encountered, sustainability plans, and the framework’s extension to other BPCI-A superbundles.

Kathleen Breda, NP, MSN, BBA, AGACNP-BC, GFN, ONP-C is the Associate Director of Orthopaedic Clinical Programs at Cedars-Sinai Medical Center in Los Angeles, California. As a certified adult, geriatric, acute care nurse practitioner, she plays a pivotal role in optimizing care for orthopaedic patients. Her expertise includes the development and deployment of clinical care programs within a complex infrastructure that optimizes patient outcomes and maximizes return on investment.

Breda oversees Cedar-Sinai’s Geriatric Fracture Program, Bone Health Program, Elective Joint Program, Skeletal Dysplasia Program, Age-Friendly Health Initiative, and the CMS Orthopaedic BPCI-A program. These initiatives have had a significant impact on patient outcomes. Her recent publication, titled “Geriatric fracture program centering age-friendly care associated with lower length of stay and lower direct costs,” was featured in the February 2023 issue of Health Services Research and highlights implementation and impact of the Geriatric Fracture Program. Commensurate with her activities at Cedars, she was recognized with the ANCC Magnet Structural Empowerment Nursing Excellence award, and Multiple Trailblazer Awards from NICHE (Nurses Improving Care for Health System Elders). She has also been a featured podium presenter at many national conferences.

Before joining Cedars, Kathleen gained valuable experience as a Nurse Practitioner and staff nurse at Massachusetts General Hospital, where she received the Yvonne L. Munn Nursing Research Award. Her diverse background also includes a 25-year business career in various high-tech companies in the Boston area.
