Pathways to Prevention: 2024 CHPSO Annual Report Released

The Collaborative Healthcare Patient Safety Organization (CHPSO) has released its 2024 Annual Report, reflecting continued advancement of its mission to eliminate preventable patient harm by transforming member-reported data into proactive strategies, safety insights, and system-level improvements. With greater engagement and stronger data integrity, CHPSO continues to be a trusted leader and national model for patient safety learning.  

Among the report’s highlights: 

  • An analysis of patterns in diagnostic errors, which finds that errors often follow predictable patterns based on clinical presentation, care setting, and patient gender. The findings offer a path toward targeted reforms that improve accuracy and reduce preventable delays in care. 
  • A recap of the year’s signal detection data, which leverages member-reported data to identify spikes in risk-related terms. In 2024, the system flagged concerning increases in workplace violence, ectopic pregnancies, and delays in care, along with ongoing risks such as falls, unclear protocols, and ineffective team collaboration. These insights empower clinical leaders to investigate early warning signs in their own organizations — shifting from reactive responses to proactive prevention. 
  • A summary of advancements in data quality among CHPSO’s growing membership, which increased by 42% in 2024. Data submissions rose by 60%, overall quality of the data submitted improved by 7%, and new Data Quality Scorecard helped members benchmark their reporting. Consistent, relationship-based outreach transformed event reporting from a compliance task into a collaborative safety effort.
