Building Safer Systems through Success Cause Analysis

Patient safety improvement efforts have traditionally focused on analyzing failures – a Safety-I approach that, while foundational, overlooks the everyday successes that enable complex care to proceed safely. This webinar overviews Success Cause Analysis (SCA), a structured, scalable method for operationalizing Safety-II principles by systematically learning from favorable outcomes. SCA adopts familiar Root Cause Analysis tools to study how and why clinical events go well, identifying promotable behaviors and system-level strengths that can be transferred across settings to foster frontline engagement, strengthen organizational learning, and enhance safety culture.

Presentation topics include:

  • Consider how Success Cause Analysis fits into a comprehensive patient safety strategy
  • Articulate the key features of Success Cause Analysis methodology
  • Identify 3 concrete actions to incorporate Success Cause Analysis into their practice

Komal Bajaj, MD, MS-HPEd is Chief Quality Officer at NYC Health + Hospitals/Jacobi | North Central Bronx, where she catalyzes quality improvement transformation across healthcare delivery. She also serves as Medical Director of Sustainability for NYC Health + Hospitals and is a Professor of Obstetrics & Gynecology at Albert Einstein College of Medicine. Dr. Bajaj is an internationally recognized physician leader who transforms complex healthcare challenges through human-centered, technology-driven approaches. Her innovative work has improved clinical care in a variety of practice environments and has informed quality/safety functions globally. A board-certified OBGYN and reproductive geneticist, Dr. Bajaj trained at Northwestern University’s Feinberg School of Medicine and Albert Einstein College of Medicine. Her work has been featured in the New York Times, NPR, NEJM Catalyst, JAMA Health Forum, and BMJ Quality and Safety. She is a Fellow of the New York Academy of Medicine and Becker’s Healthcare Review 2024 and 2025  “Patient Safety Expert to Know.”
