Reducing preventable patient harm is an ongoing challenge for the healthcare industry. For example, in Canda, patient safety
incidents account for $2.75 billion dollars annually, with 1 in 10 Canadians reporting that they have experienced an event themselves.
In 2019, the Mackenzie Health (MH) system conducted an organizational safety culture survey, which revealed an opportunity for improvement in patient safety practices. This was exacerbated by anecdotal concerns that safety events were being underreported across the health system. In response, MH began a journey toward adopting a zero harm approach to reducing safety events for our patients. This webinar presents an organizational approach to prioritizing the prevention of harm across our inpatient setting and shares our findings and key insights from its zero harm journey.

Mary-Agnes Wilson, RN, PhD
Interim President & CEO, Mackenzie Health
Dr. Mary-Agnes Wilson is the Interim President and CEO of Mackenzie Health, a major community hospital system north of Toronto. With a career spanning clinical nursing, research, academia, and executive leadership, she is a strong advocate for patients, families, and health care providers.
She has led several firsts in Ontario, including the creation of the Provincial Birth Defects Registry, achieving Canada’s first Magnet designation for an acute care hospital, and guiding the opening of Cortellucci Vaughan Hospital. She also directed Mackenzie Health’s COVID-19 response and positioned the organization as the first in Canada to fully adopt high reliability organizing principles.
Dr. Wilson holds a BScN and MN (Administration) from the University of Toronto and a PhD from McMaster University. Known for her leadership and expertise, she continues to champion quality improvement and the adoption of best practices in healthcare.
