Collaborative Risk Review™ – A High-Reliability Method for Conducting Case Analyses

August 14, 2024 | 10:00 - 11:00AM PT

Collaborative High Reliability™ is the only evidence producing method known for proactively reducing risk. Using concepts that are based in commercial aviation’s ASAP program, tied with the latest in socio-technical science, participants will join Paul LeSage in deconstructing a risk incident using a Collaborative Risk Review™, the method used by all organizations that have been qualified in Collaborative High Reliability.

Paul will also briefly cover some of the remarkable statistics associated with the increase in risk reporting at hospitals and EMS organizations in Boston, Tampa, Toronto, and Texas, which has given these entities their first look into what used to be unreported risk – the risk that lies “under the water” that we rarely get to see.

Presentation topics include:

  • Conduct additional research on how high reliability is evolving as a socio-technical science.
  • State why the term “Preventable” cannot be used as the conclusion of any analysis without specific metrics.
  • Describe why FMEA and RCA are “second generation” methods that don’t lend themselves to proactive risk mitigation.
  • Understand how a robust Fact Gathering and Facilitation process contribute to better risk reporting.
  • Understand how a Collaborative Just Culture™ is a required element if looking to increase front-line reporting and participation in risk analyses.
